🏡 Self & Family Managed Care (SFMC) Funding in Manitoba

Taking care of loved ones at home can feel overwhelming. Manitoba’s Self and Family Managed Care (SFMC) program allows families to take control of home care funding so they can hire and manage caregivers directly.

🎯 Eligibility Criteria

🏠 Manitoba Resident

• Must live in Manitoba
• Must be registered with Manitoba Health

🧑‍⚕️ Enrolled in Home Care

• Already a Home Care Program client
• Eligible for attendant or homemaker services

🛋️ Daily Living Support Needed

• Requires help with: dressing, bathing, grooming, eating, mobility, or light housekeeping

🛡️ Need to Remain at Home

• Care is essential to live safely at home

🤝 Beyond Existing Supports

• Current family/community supports are not enough

📋 Case Coordinator Assessment

• A Case Coordinator will assess needs
• Funding amount is based on assessment results

⚠️ Note: Family members are not usually approved as paid caregivers, except in special circumstances.

📝 Step 1 – Apply

• Complete WRHA Home Care Referral Form
• Call WRHA Intake: 204-788-8310

👩‍⚕️ Step 2 – Assessment

• Social worker home visit
• Client + family/POA interview
• Wait time: about 3 months

📬 Step 3 – Approval Letter

• WRHA sends letter with:
  • Approved hours of care
  • Services covered

💳 Step 4 – Set Up Funding

• Open a separate bank account
• WRHA deposits monthly funding

👥 Step 5 – Manage Care

• Option 1: Hire caregivers directly
• Option 2: Partner with an agency (e.g., Comforts of Home Care, Home Instead, Right at Home)

📊 Monthly Requirements

• Track caregiver hours and wages
• Keep timesheets, payroll records, receipts
• Submit reports to WRHA as required
